We are a small medical office and do most of our billing on HCFA-1500 claim forms. I mean, basically, they are paper printed with the proper fields for any insurance billing. They print, they mail, they do what they are supposed to.

As described, prompt shipping, good quality, works perfect for getting claims paid.

Same forms used at medical practices normally purchased at big box stores for close to double the price! Excellent quality, no issues with printing. Can't beat the deal on Amazon unless you find a local independent supplier who will price match. Fortunately, I did, but this will be my back-up.

the form format did not allow the form to be printed properly, the size is different. We tried a different form form another company and it was printed properly.
