Positive and negative reversed!
Talk about a disaster waiting to happen. You have the current capacity of 6 batteries in parallel with reversed wiring. I couldn't wait for a replacement so I put black heat shrink over the positive red wire and cut the banana jacks off to prevent future catastrophe.

Insulation not covering the solder joints
Insulation that was suppose to cover the solder joints was loose. Had to redo the insulation on the solder joint.
